Patterns of Meniscal Tears Associated with Ruptured Anterior Cruciate Ligament: A Retrospective Study



Abstract
The correct identification of a meniscal tears associated with an ACL tear is often a major determinant of subsequent management. Controversy still exists, but a factor regards as most important for the fate of the knee is the state of menisci,
both at the time of injury and at follow-up. Furthermore, the pressure of meniscal lesions with an ACL tear may contribute to increased instability and alterastions of the loocal transmissions forces across the knees.
The purpose of this study is to define the nature of the meniscal tear encountered in association with an ACL tear, with and without an associated collateral ligamentous injuries. In the period january 1991 to June 1995, 93 ACL injured knees were
reviewed retrospectively after arthroscopic surgery had been performed.
@ES The results were follows;
@EN 1. In 39 cases(41.9%), the rupture of ACL was associated with the medial meniscus tear; in 15(16.1%), the lateral meniscus tear; in 12(12.9%), both menisci tear, and in the remaining 26(28.0%), the meniscus was intact state.
2. The most frequently associated pattern of meniscal tear with rupture of ACL was the 12 cases of buckethandle tear of medial meniscus, followed by the 10 cases of peripheral tear of medial meniscus.
3. The most frequent site of meniscus tear with rupture of ACL was posterior horn of medial meniscus(19 cases), followed central portion of medial meniscus(13 cases).
4. At the medial meniscus, the average time form injury to arthroscopy for bucket-handle tear was 24 months, for peripheral tear 8 months, and at the lateral meniscus, the average time from injury to arthroscopy for central tear 5d months, for
peripheral tear 1.8 months.
5. so, early arthroscopy shlould be recommended in patients, in whom an ACL tear is suspected on clinical and MRI. If the meniscal tear is fo9und at the time of arthroscopy, then this should be treated.
